The mapper treated every device as fully connected, so on a coupling graph
with more than one connected component it could emit a mapping that routes a
two-qubit gate across qubits with no physical path — a wrong result rather
than an error. Model unreachable qubit pairs explicitly, place and route per
connected component, and reject unmappable circuits instead of producing an
invalid mapping.

- Device: add `unreachableDistance` sentinel honored by `getDistance`/
  `hasPath`/`getShortestPath`; fix BFS parent init and all-pairs paths so
  disconnected components stop fabricating connectivity; replace
  `Device::file` with `tryFile` returning `llvm::Error`.
- Mapper: decompose device and virtual interaction graphs into components;
  add a best-fit-decreasing component seed for `auto` placement; drop seeds
  whose interactions span components (`findUnroutableInteraction`) and fail
  with a precise diagnostic when none is routable; account for
  `unreachableDistance` in greedy centrality; reject >2-qubit gates.
- Harden two pre-existing crashes: guard the unset-device dereference in both
  mapping passes, and fix the qubit-overflow bounds check that wrote past
  device-sized tables. Add SABRE reachability asserts.
- Tests: disconnected and invalid-topology coverage plus crash-fix
  regressions.

Thomas Alexander's PR NVIDIA#4753 (commit 4ecd349, "Support disjoint and sparse
device topologies in the qubit mapper").
